East Dunbartonshire Community Mental Health Team (CMHT)

East Dunbartonshire Community Mental Health Team (CMHT) will aim to work with people with complex mental health problems and associated risks who typically require longer-term care and treatment, multidisciplinary engagement, and with the capacity to provide assertive care and to manage higher levels of risk.

Typically CMHTs will work with two broad groups of patients.

People with conditions that require interventions of weeks or months, with discharge on completion of the intervention. People who require ongoing treatment, care and monitoring for prolonged periods, who should be managed within the recovery model with an expectation of improved functioning which may enable discharge.

CMHTs will work in partnership with patients, families and carers, primary care and other agencies to design, implement and oversee comprehensive packages of health and social care where needed, to support people with complex mental health needs.

In conjunction with Crisis Teams, CMHTs will operate a duty system available 7 days per week. The minimum core working hours of CMHTs are Monday to Friday 0900 to 1700.

CMHTs will aim to deliver services in a suitable environment that is accessible to the individual (and their carers). This will include team base, GP surgery, the patients’ home or other appropriate community venues.

Community Mental Health Team key functions:

Assessment – all accepted newly referred people will have their health and social care needs assessed by the CMHT.

Interventions for people with complex and acute mental health needs – people who have been assessed as having complex mental health needs are likely to require a range of services and interventions in order to promote recovery and to prevent deterioration, including the provision of specific or tailored treatments, the monitoring of the effectiveness of these treatments and the monitoring and effective management of unwanted side effects. Services offered in CMHT are Nursing, Occupational Therapy, Psychology including Cognitive Behavioural Therapists, Mental Health Practitioners and Psychiatry.

The Peer Support Service - Community Mental Health Team (CMHT-Larkfield)

The Mental Health Peer Support Service within Larkfield Community Mental Health Team, offers short-term input to patients who feel they would benefit from support from a person with lived experience of mental health problems.

The main aim of Peer Support is to inspire hope that recovery is possible. It gives a person a chance to speak about their experiences of recovery to someone who is also on their journey of recovery, and be met with empathy and non-judgement. Experiencing mental health problems can be very isolating and stigmatising, and we discuss such experiences.

This support looks different for every person: some people prefer conversational-based support, sharing experiences of mental health recovery, whilst others prefer support to achieve their goals, eg introductions to local services or community groups.

Many people opt for a combination of both. We very much let the person we work with decide how we use the time together. We can also offer WRAP (Wellness Recovery Action Planning) which focuses on relapse prevention, and empowerment to take personal responsibility for recovery.

We offer meetings within Kirkintilloch Health and Care Centre, or within community venues, depending on a person's preference.

Promoting and supporting mental well being and prevention of relapse - raise awareness and promote positive attitudes to mental health issues.

Access into the Community Mental Health Team should be through your GP or Social Services.
